Immediate implantation — when is it appropriate and what are its advantages?
Immediate implantation represents a modern approach in dental implantology, where the implant is placed immediately after tooth extraction — within a single surgical procedure. This approach offers numerous advantages for both the patient and the clinician.
What is immediate implantation?
Unlike the classical protocol, where 3–6 months of bone healing is awaited after extraction, immediate implantation allows the implant to be placed directly into the fresh extraction socket. This significantly shortens the overall treatment duration.
Indications
Immediate implantation is particularly suitable in these cases:
- Single-rooted teeth with sufficient bone volume
- Teeth with chronic inflammation, where the bone is stable
- Anterior region — where aesthetics and speed are important
- Patients with good overall health status
Contraindications
Not every case is suitable for immediate implantation:
- Acute infection around the tooth
- Insufficient bone volume (less than 3 mm of buccal lamina)
- Uncontrolled diabetes or immunosuppression
- Bruxism without adequate therapy
Advantages for the patient
- Shorter treatment duration — no waiting period for healing
- Fewer surgical procedures — extraction and implantation in one
- Preservation of soft tissues — better aesthetic result
- Immediate provisional solution — patient does not leave edentulous
Key success factors
The success of immediate implantation depends on several factors:
Primary stability
The implant must be sufficiently fixed in the bone (minimum torque 35 Ncm). This is achieved by correct selection of implant diameter and length and utilization of apical and palatal bone.
Correct positioning
The implant is placed slightly palatally/lingually, not in the center of the extraction socket. This ensures sufficient buccal bone for long-term stability.
Gap management
The gap between the implant and the buccal wall (so-called jumping distance) is managed with bone graft or biomaterial. A distance exceeding 2 mm is critical.
Modern approaches
Contemporary implantology uses digital planning (CBCT, intraoral scanners) for accurate diagnosis and outcome prediction. 3D guided surgery allows implant placement with millimeter precision.
